EMSPCR.app is state-agnostic by design. RIEMSIS, RIDOH, and Comstar are the Rhode Island pilot's consumers because Providence Fire is the pilot agency. Every other state plugs in its own state NEMSIS submission hub, its own state DOH surveillance endpoint, and its own billing partner through agency-level configuration. The capture layer, click-through protocol engine, NERIS submission, and HIPAA audit log are identical in every state.
EMSPCR.app posts to the federal NERIS v1 endpoint at api.neris.fsri.org with full schema-version headers. The NERIS payload is auto-derived from the ePCR captures — no double-entry. Fire-side sections (100-series structure fire codes) and EMS-only sections (300-series rescue codes) branch automatically based on incident type. Built-in submission summary verifies what was sent before it reaches the wire.
Your billing partner (Comstar in Rhode Island; agency-configurable for any partner in any state) receives signed PCS, HCPCS, origin-destination modifier, ICD-10, and GPS-precise loaded mileage in a single consistent payload derived from the same captures the EMT entered for the clinical record. Origin-destination modifier auto-assembled with invalid-combo blocking. Level-of-service validator catches BLS-with-ALS-interventions mismatches before claim export. PCS expiry tracked. The result: fewer denials, faster recoupment.
The chief complaint and provider impression activate the relevant state-protocol as a click-through care matrix (RI Statewide EMS Protocols loaded for the Providence pilot; the engine is state-agnostic and configurable for any state's protocol set). Each tap inside the protocol auto-timestamps the treatment, procedure, or medication, with live dose calculators, and fans the click out to NEMSIS, your state's EMS submission hub, NERIS, your billing partner, narrative draft, and STEMI / Stroke / Trauma cascade triggers. EMSPCR.app embeds HIPAA-tier Daily.co video sourced from Instar Health's telehealth stack. No commercial EMS EHR — ImageTrend, ESO, Zoll, Beyond Lucid, EmergencyReporting — ships native telehealth. Unlocks Online Medical Control (paramedic-to-base-physician video), Mobile Integrated Healthcare consults, pre-arrival cascade video to the receiving emergency department, and family-on-call.
Compliance is the schema. NEMSIS 3.5.1 element mapping, your state's EMS submission composition (RIEMSIS in Rhode Island; any other state's NEMSIS hub elsewhere), your state DOH surveillance flag derivation, federal NERIS payload, and NHTSA DOT crash subset are defined alongside the captures that fill them. A submission cannot be malformed because there is no path from capture to wire that doesn't flow through validation rules.
